Please submit:

• completed and signed ABC Producer Profile
• signed and dated ABC Producer Agreement
• signed and dated W-9 form
• copies of your licenses for all states in which you will solicit business
• a copy of the declarations page from your in-force insurance agent’s errors & omissions policy

In addition to being contracted with ABC, you will need to be appointed with ACE to sell the limited medical and hospital deductible insurance plans.

Commissions are paid on the 5th of each month as earned for premiums received in the prior month.
